IMPORTANT NOTICE TO ALL PATIENTS

The Practice would like to make all patients aware that the Health Authority is currently contacting individuals whose correspondence has been returned marked "No longer at this address".  This may occur if a patient has not responded to a letter or has not taken the necessary action following receipt of correspondence.

It is essential that patients respond promptly to any letters received from the Health Authority.  Failure to do so may result in patients being removed from their registered GP Practice list.

If we are informed of an impending removal, we will also make contact with you to confirm your current address, so please respond.  Please also remember to update your details when they change, even if you have not had any contact such as this.

We kindly ask that you share this important information with your friends and family to ensure that they are aware of the situation and understand the importance of responding to correspondence from the Health Authority.
